Your Role: Social Worker / Admissions Coordinator

This is more than a clinical role -- it's about being a trusted guide, a compassionate listener, and a tireless advocate for those who need it most.


What You'll Do:

  • Be the calming presence and emotional support for residents and families

  • Lead thoughtful assessments and build care plans that truly matter

  • Step in during crises with clarity, care, and compassion

  • Collaborate with a rockstar interdisciplinary team to provide whole-person care

  • Connect residents and families with vital services and community resources

  • Champion resident rights and quality of life

  • Keep clear, thoughtful documentation of every step on the journey

  • Assist with the admission process


You Bring:

  • At least 2 years of Long-Term Care experience

  • CSW preferred -- but your heart and skillset matter most

  • Integrity, empathy, and a truly positive energy

  • Strong communication and advocacy skills

  • A commitment to treating every resident like family
